For cataract patients with regular astigmatism, toric IOLs provide the potential for better uncorrected visual acuity than they have ever had. For surgeons, though, toric lenses pose a challenge because even small errors in an IOL’s position may significantly affect the patient’s uncorrected visual acuity—more so than they would with a spherical IOL.

Aspheric Monofocal IOLs such as the Tecnis, AcrySof IQ, and SofPort AO (Bausch & Lomb) are designed to reduce overall spherical aberration, but each lens does this in a different way. The IOLs are specifically designed to neutralize all corneal spherical aberration in the average cornea. In a young eye, negative spherical aberration of the crystalline lens balances positive spherical aberration of the cornea . Young eyes have little spherical aberration, and light is sharply focused on the retina, producing a quality image and good functional vision.

Aspheric IOLs

Traditional intraocular lenses have a spherical optical design, meaning the front surface is uniformly curved from the center of the lens to its periphery. Though a spherical IOL is relatively easy to manufacture, this design does not mimic the shape of the natural lens inside the eye, which varies in curvature from center to periphery. In other words, the eye's natural lens is aspheric ("not spherical").

Why is this important?

A spherical intraocular lens can induce minor optical imperfections called higher-order aberrations (HOAs), which can affect quality of vision, particularly in low-light conditions such as driving at night.

Premium aspheric IOLs, on the other hand, match more closely the shape and optical quality of the eye's natural lens, and thereby can provide sharper vision — especially in low light conditions and for people with large pupils.

Popular aspheric IOLs that are FDA-approved and available world include: Tecnis Aspheric (Abbott Medical Optics), AcrySof IQ (Alcon), SofPort AO (Bausch + Lomb), and Aspina (Zeiss).

The most common complication of cataract surgery is clouding of the part of the lens covering (capsule) that remains after surgery, called posterior capsule opacification. If the cloudiness affects your vision, you may choose to have a laser surgery called Nd:YAG posterior capsulotomy to correct this problem.

What To Expect After Surgery

The person may wait in the outpatient surgery area or the doctor's office for 1 to 2 hours after the procedure so that he or she can have the pressure in the eye (intraocular pressure) checked. Intraocular pressure (IOP) is the pressure caused by the fluid inside the eye that helps keep the shape of the eye. You should not feel pain after the surgery.

Why It Is Done

After cataract surgery, some people notice cloudiness (sometimes called aftercataract) after several months or years. In some people, it can become very dense and cause as much or more vision loss as the original cataract.

The decision to have this procedure is based on the same criteria as the decision to have the original cataract surgery:

Vision problems are affecting your work or lifestyle.

Glare caused by bright lights is a problem.

You cannot pass a vision test required for a driver's license.

You have double vision.

The difference in vision between your two eyes is significant.

Currently the goal of glaucoma surgery and other glaucoma treatment is to reduce or stabilize intraocular pressure (IOP). When this goal is accomplished, damage to ocular structures — especially the optic nerve — may be prevented.

When Is Glaucoma Surgery Needed?
